An NDIS plan outlines the support and funding you receive through the National Disability Insurance Scheme (NDIS) to help you pursue your personal goals and live your best life. What’s in Your NDIS Plan?
Your NDIS plan is based on discussions during your planning meeting and contains five key sections:
1. Information About You
This section includes:
Your disability and its impact
Your daily activities
Where and how you live
Who provides informal support (like family or carers)
2. Family and Friends
Your plan will acknowledge the unpaid support you receive from family or friends, which plays an important role in helping you work toward your goals.
3. Services and Community Groups
Here you’ll find:
Services provided by local and state governments
Access to community programs, such as public transport, libraries, and support groups
These services are not funded by the NDIS but are still essential for your wellbeing.
4. Your Goals
Setting goals is a key part of the NDIS. Your plan will include:
Short-term goals you want to achieve within your current plan
Long-term goals for your life

The funding is based on what’s reasonable and necessary for your unique situation, factoring in informal support and government/community services.
